How PharmaSim scenarios are built, what skills they assess, and how feedback is generated.
What PharmaSim is
PharmaSim is an educational pharmacy simulation. You meet a virtual patient, gather information through the consultation, make a professional decision, and receive an honest debrief. Scenarios are generated dynamically — each simulation produces a new patient with varying demographics, symptoms, medications, and risk factors.
How scenarios are created
Every scenario is built around a real-world pharmacy situation: a patient walks in with a complaint or request. The scenario then presents a sequence of decision points a pharmacist would face — from the initial greeting through assessment, recommendation, and follow-up counseling.
- 1Patient profiles vary: age, sex, comorbidities, medications, allergies, and behavior change each time.
- 2Clinical factors are randomized: the same complaint can hide different red flags depending on the patient.
- 3Difficulty tiers escalate complexity: Basic (one factor), Intermediate (multiple factors), Advanced (hidden red flags, polypharmacy).
- 4Answer choices are designed to be equally plausible — the correct response depends on the specific patient, not on the wording of the option.
What skills are assessed
Patient assessment
Gathering a complete history before recommending anything — duration, severity, prior treatments, comorbidities.
Safety & red flags
Identifying contraindications, dangerous interactions, and symptoms that require referral rather than an OTC product.
Clinical reasoning
Weighing multiple factors — age, pregnancy, kidney function, current medications — to choose the safest and most appropriate action.
Communication
Explaining decisions to the patient, giving clear instructions, and knowing when to refer to a prescriber.
Why patient assessment matters
The most common pharmacy error is recommending a product before fully understanding the patient. PharmaSim trains you to pause, ask, and assess before you act. In many scenarios, the correct decision is not to dispense a medication — it is to ask another question, check an interaction, or refer to a doctor.
How feedback works
After each decision, you see the consequence of your choice. After completing a scenario, a full debrief breaks down your performance across four competency dimensions: safety, clinical reasoning, medication assessment, and communication. The debrief explains which red flags were present, what questions you should have asked, and why each option was better or worse.
Difficulty levels
One main clinical factor. Focus on identifying the right OTC product and basic safety checks.
Multiple factors — comorbidities, contraindications, drug interactions. Requires additional questioning before recommending.
Hidden red flags, polypharmacy, multiple risk factors. Requires full clinical reasoning — the correct answer is rarely obvious.
Deliberate practice
PharmaSim follows the principles of deliberate practice: realistic scenarios, immediate feedback, and repetition. Each simulation generates a new case, so you can practice the same skill area with different patients and never memorize answers.
